Welcome to Women & Infants Fertility Center. We are excited to work together to help you achieve your fertility goals. To help you during this time, we have put together the following information and helpful references for your first fertility consultation.

Please keep in mind that this is general information, and your treatment may vary based on your history or response to previous fertility treatment.

What you should bring to your first appointment

Please make sure to arrive 15 minutes before your scheduled appointment and bring the following:

A completed infertility treatment new patient packet (Please Note: This form is fillable.)
A completed female gynecologic care new patient packet (if applicable)
Medical records and important health history. You may also have your health provider fax your medical records to us at (401) 453-7598
Photo ID and insurance information card
All X-ray images
A list of current medications

If you know which treatments or services you’re interested in at Women & Infants Fertility Center, we encourage you to review the consent forms for the treatments and bring in completed copies for your first appointment.

Policy for missed appointments

If you need to reschedule or cancel an appointment, please give us at least 72 hours notice. If you do not give us 72 hours notice, or you do not show up for a scheduled appointment, you may be charged a $30 fee for a missed office visit or $100 for a missed procedure or testing appointment.

We realize things can happen that are out of your control, so you will not be charged for your first missed appointment. However, you will be charged for any subsequent missed appointments.

We also ask that you please show up on time for your scheduled appointment. To help you, we will call to confirm your appointment date and time. We will also remind you of any referrals or authorizations that you will need for the appointment.
